Applications
must be received by February 1st (1½ years prior to the first fellowship
year). We participate in the National Residency Match Program.
Requirements:

(please type or print neatly).

Personal
statement.

Three
letters of reference (1 from the Director of the Department you most
recently served) sent directly to Dr. Salata.

Curriculum
Vitae (Chronological list of activities including schools, degrees,
graduation dates, academic honors, publications and any special
training).

Enclose
a stamped, self-addressed post card if you wish acknowledgement that we
received you application.

Training
in an approved U.S. based residency program.

Personal
interviews are required upon invitation only. We interview during the
months of February, March and April.
